  Requirements:
  
  • 
    
  • Social Work degree (BSW or MSW) required
    
    
  • Clinically Licensed Social Work - highest licensure in the state - required
    
    
  • Minimum of three years of social work experience, preferably in a healthcare setting
    
    
  • Knowledge of the geriatric population, case management, and dementia is preferred
